To become a member, your agency representative
will need to submit our Partnership Application Form.
Upon submission, the Partnership Committee will contact you to complete the process.
 Eligible applicants will be required to:
1) Submit signed copy of the Partners Individual Networking Agreement
to the Partnership Committee.
2) Coordinate with the PHC Lunch Rotation Committee for scheduling
presentation dates with hospitals
