About the Event
Presentation: "The GUIDE Model" presented by Jennifer Wagenhoffer, MS, OTR/L, CLT, At Home Harmony. Learn more about the voluntary nationwide model test designed to assist unpaid caregivers; scheduled to last for 8 years. The GUIDE Model is centered on providing comprehensive, coordinated dementia care with the goal of enhancing the quality of life for individuals with dementia, alleviating the burden on their unpaid caregivers, and facilitating the ability of individuals with dementia to stay in their homes and communities.
Partner's Breakfasts are held the 1st Friday of the month from 8:30 – 10:00AM. There is no charge for Partner's members. $15.00 fee for guests. All attendees will receive a Certificate of Attendance Form good for 1 hour of education. RSVP is required one week prior to the meeting.
